To edit or update your current LifeShare Donor Registry information, please provide us with
your Registration ID in the available field.
Only those who have previously registered online have been issued a Registration ID. If
you enrolled through a tag agent, DPS or mail-in form you will not have a Registration ID,
however, you may register online now to obtain an ID. This
will allow you to review your
record and make any changes in the future.
If you have registered online, and have misplaced your Registration ID please
complete this form and your Registration ID will be
emailed to you.
