LifeShare Donor Registry Information
This area of the LifeShare Donor Registry will answer common questions that you might encounter while registering online to be a donor. These questions and answers are specifically related to the online donor registry.

Question: Who can donate?
Answer: Anyone can be a potential organ, eye and tissue donor, from newborn to senior citizen. Eligibility is determined on a case-by-case basis at the time of death and may be affected by medical/social history, cause of death, etc.

Question: Where can I register to be an organ, eye and tissue donor?
Answer: Registration can take place at several different locations. You can register to be a donor online, at tag agencies when you get your license or state ID card and at some health fairs. Signing up only takes a few minutes in any place you register. If you would like to sign up online start at www.lifeshareregistry.org , select the "Register Now" button, and follow the directions.

Question: Can I choose what is donated?
Answer: Yes, on the registry you can list any specific exclusions you have to any organ, eye and tissue you do not want to donate. You may also choose if you would like to just donate for transplantation or whether you would like to donate for transplantation and research. The decision is up to you.

Question: If I sign up to be an organ/tissue donor at a tag agent or the Department of Public Safety (DPS), do I also need to sign up online?
Answer: If you choose to be an organ, eye and tissue donor at a tag agent or DPS, when you get your license or state ID card, you will be added to the registry automatically, however, we encourage you to sign up again as this might allow you to tell others about the online registration process.

Question: If I move, change email addresses, telephone number or change my mind about something concerning donation where do I make this change?
Answer: If you have signed up with a tag agent/DPS you would need to call DPS or a tag agent to make changes. If you sign up online, after you go through all the steps, you will be given a registration ID number. This number, along with other personal information, will allow you to log into the registry site and make changes through the My Profile area.

Question: Do I need an email account to register online?
Answer: Yes, you need an accurate email account so you can receive your registration ID number and confirmation of your registration. You can sign up for free email accounts at yahoo.com, hotmail.com and mail.com.

Question: Why is the signature card needed?
Answer: Oklahoma state law requires that your signature be on file, if you register online.

Question: How does my signature become apart of my file or profile?
Answer: A signature card will be mailed to you after your completion of the online registration form. When LifeShare Donor Registry receives your signature, they will scan it into your profile and your name will be added to the registry.

Question: Am I officially on the registry only after my signature is attached to my profile?
Answer: Yes, you can only be added to the registry after your signature is attached with your profile. Anyone added to or removed from the registry will require a signature for security reasons.

Question: What happens if I am under the age of 18 and register online?
Answer: Everyone who signs up online will be required to return to LifeShare Donor Registry a postcard with their signature. If you are under the age of 18 this card will also need your parent or legal guardian's signature. Oklahoma law requires your parent or legal guardian to sign a consent for donation until you reach the age of 18. However, once you turn 18 years of age, and if you are already in the online registry, your registration will change from indicating an intent to be a donor to an actual consenting adult donor.
